Should O2 Sensors be replaced based on age or when they throw a code? Dealer says wait for the code.

Sounds like a recipe for me to make a mistake!

I replaced mine at 250k kms because some website fed me some garbage about them deteriorating and slowing in response over time. I noticed no difference between the old and the new. Even watching them with my scanner, there was no change in speed or voltage that was noticeable. I would wait until they throw codes.

you can check o2 sensor with a volt meter.
