Help identifying a part...

need help finding out what this part is called. I'm sure its a dealer part. I epoxied about 2 years ago and just gave way today.

Passenger side, hose comes from water outlet to the T then 1 goes to heater water valve and other goes to heater core i believe.

Those would be molded heater hoses. You either replace with a barbed fitting or see if it’s available somewhere in one piece. Such as the dealer.
