Intermittant Starting Difficulty

I have a 2002 XLT V8 with about 46K miles. For about a year now, off and on, I have had problems getting it to start and stay running! I believe that it only happens when its cold out, but not any specific time of day. It will crank like it wants to start, but it won't actually fire. It takes a lot of TLC and patience to get it going. After turning the key and smashing the gas pedal to the floor for about 3 seconds it will start and then I have to give it gas continually until it stays running. Once it is running for a minute and I take off, it is perfectly fine. It will then continue to run fine for a long time and then I'll have the same problem. This is annoyiny! Any suggestions? Thanks!
