seat belt modifications to minimize back pain?

I have a 2006 EB Explorer. I'm wondering if there are any seat belt modifications for people with low back pain. My wife has a lot of back pain and the lap part of the belt makes it worse. I had an 86 Park Avenue with a separate lap belt and shoulder belt retractor. With that, it allowed you to be belted but shift around in the seat easily. Anything like that for an explorer or anyone know of companies who could make something work?

if you go to a ford dealer they can order you i think for free and seat belt exstention that might help. my mom used that in her Bronco and now she got one for her Focus.
