1997 Mountineer Heat Issue (not sure if it's the blend door)

My girlfriends 1997 Mountineer's heat comes and goes. Sometimes it works fine, others it wont. Sometime it will be blowing warm air, than just switch to cold. In the summer her A/C will start blowing warm air randomly. Is it the blend door? Just the actuator? She said sometimes when she changes from hot to cold she can hear a grinding noise. I haven't heard it when I've been in her car, but I really need to get something done here cause it's been damn cold here in Iowa.

Are you saying that after the engine has warmed up and the vents are blowing hot air. The air will go from hot to cold by itself?

If so that does sound like a blend door issue. A grinding noise would indicate the actuator is broken.
