The simple answer is: Swap transimissions, add transfer case, swap entire front axle assembly. That is, of course, a very simple listing of what needs to be done. Actually doing it is going to be extremely complicated and time consuming.
I wouldn't say it can't be done, I've seen Chip Foose turn a Cadillac into a Hot Wheels car. You need to decide if it is worth the $$ and effort, when you can just buy another truck.
