Will a 422 work with an otherwise stock 4.0 OHV? Or does it need the heads/intake/exhaust work done as well. And by work, I mean, realiabiliy run without operating issues. Thanks

You need different valve springs.
To install the springs, some seat work needs to be done.
Apparently, there is a tool for this that can be used with the heads on the truck, but for all that work, its easier to pull the heads.

I don't know for certain, but the cam manufacturer says tuning is required with the cam install. I think the main thing is to be able to turn the idle up.

You can get the cam in without pulling the motor, but getting the harmonic balancer bolt off it painful. If you do the cam swap, make sure you loosen this bolt before you take anything apart.
