(CNN)An automotive assembly line robot mortally injured a worker at a car factory in Germany this week, a prosecutor's office said.

The man, 21, was installing the robot at a Volkswagen assembly line on Tuesday in Baunatal, which lies next to the city of Kassel, local newspaper the Hessische Niedersaechsische Allgemeine reported, citing a VW spokesman. The robot gripped him and pressed up him against a metal plate, crushing his chest.

Looks like someone broke law #1

In the story I read, officials were still trying to decide if charges would be filed.

Questions come to my mind.

Was it a malfunction of the robotic arm?

My .02 , Since several commands would have been executed for the accident as it was to reported to have happened, it sure does not sound like a malfunction to me.


Could an angry co worker have written an attack program?

On a serious note it seems like Someones LOTO (Lock Out Tag Out) procedure or the commissioning protocol procedures were not followed.
