broken od servo cover,5r55s 03 explorer

After a routine 5r55s repair, typical od band burnt,drum damaged, od pinions overheated and ruined. new parts replaced and od bushing, after a hundred miles or so the od servo cover cracks and loses fluid.
Has anyone seen this and know the cause?, cover had fresh break marks, no older cracks found.
Engaged and shifted perfect after repair.

I have not seen the servo covers themselves crack on this transmission, it must be a defect somehow or something damaged it at one time.

I wouldn't think it would happen again.
