P0104: BP sensor voltage out of range. Check the connectors and wiring to the Barometric Pressure sensor. Could also be a bad sensor.
P0402: Excessive EGR flow rate reported to PCM - often caused by a bad EGR flowrate sensor. On most 96+ Fords, the flowrate sensor is a DPFE sensor.
P0141: Heater Circuit Malfunction (rear oxygen sensor if there is just one, passenger side rear oxygen sensor if there are two). Wiring to oxygen sensor heater is bad or heater element in sensor is bad.
This one will probably be mounted on the intake manifold or somewhere else in the engine compartment. There should be two hoses leading to it from the EGR tube.
