Loose PCV Valve at Valve Cover

I was smelling some burnt smell when I parked my 04 Sport Trac in the garage. Saw that a little bit of oil is getting on to the exhaust manifold on the driver side. Saw that PCV valve is loose at valve cover. PVC valve is not broken. PVC valve was replaced at 100000 miles, now 121000 miles. No CEL. Does this mean a valve cover replacement or a grommet that broke and needs to be replaced?

Loose as in fell out?
Did you replace it?
Is it the correct one?
Grommet bad? Not on my 2001 with 214,000 replaced twice by me!

1. It is still be held by the other hoses.
2. Replaced at 100,000 miles.
3. Yes, it is a Motorcraft.
4. Seems like a missing grommet. Is there a replacement?
