P0153 Bank 2 Sensor 1 Back again

1996 4.0 EXPLORER 2WD 250,000 miles; I replaced the 02 sensor on the driver side front a short while ago, about a month. CE light came back on today and scan reads p0153.bank2 sensor 1. If this is the same sensor, why would it fail twice? Seems to run just fine

Bought it at Oriellys autp parts. I will have to check with them tomorrow to see and try and swap it out

Well, I took it out and in one of the slits where the air goes through it was half way clogged with a white ashy type of material. Might of blocked air flow off. Any ideas what this white ashy stuff is? Oriellys let me swap it out for a new one

I am not 100% sure but is white not mean its running to lean??
