I am trying to find a good video online of a PCV valve replacement for my 2004 Ford Explorer Limited. Thanks for your help!

Woo hoo!

yes, you got it. That's the one.

Does it look like it needs to be replaced?
Should it be rattling like that?
I also noticed the resovoir it is screwed into did not have oil up to the upper part of the reservoir. Is that ok or is it low on oil?

pull the dipstick and check the oil, you wont be able to see the oil level from the top of your engine, if you think the pcv valve is bad just replace it they are not too expensive.

It's working fine if it rattles but since it's very cheap and needs to be replaced every 100k better to replace it with a new one and forget about it until the next 100k. There will be traces of oil in the valve cover (oil level is not checked at this area since oil doesn't stay here) Most of the issue is oil getting into the pcv valve elbow hose towards the throttle body and over time the hose gets ruptured)

GREAT EXPLANATION! PERFECT! Thank you! I'm at 108,000. I will replace the PVC. I am also looking to replace the spark plugs. Any advice anyone has on accesing and replacing the spark plugs on my 2004 Explorer Limited would be GREATLY appreciated!!!

Just thought I'd add, If you pull it, replace it. It has a locking nib that gets sheared off when you pull it out. You can see it in the last picture you posted. Pretty sure manual also says the same.
