I have a '94 explorer w/ manual transmission. When driving for approximately 30 minutes, it becomes very difficult to shift, even hard to get out of any gear. I took it in for service, they added clutch fluid and bled the system; this worked for about 1 1/2 months and then the problem came back. Clutch fluid level is fine. Shifts fine until driving for about 30 minutes and everything warms up. Any thoughts on the problem and suggested fixes?

Your slave cylinder is on its way out. You could always try bleeding it again, since that helped for a while the first time.
