Without a tach to see what the motor is doing, kinda makes it a little more difficult. I'm wondering if the torque converter is locking up when it should and if you're actually shifting into OD. Personally, I've never had good luck with the A4LD, I would have already swapped a manual transmission into that van and been done with it. Most common issues of it not shifting are, vacuum modulator, I know you replaced it but that doesn't mean it's good, it means it is untested. Valve body or a pump issue, marginal fluid pressure.
