1996 Explorer 4.0 OHV
I have a 4 door 1996 ford explorer xlt 2wd automatic v6, and the check engine light isn't on, but around 55mph it starts stumbling/running rough.
I hooked up my scanner CP9125 and it gave me 2 codes -- p0141 and p0147 p/d on both of them.

Im hearing o2 sensor, but both of them together can mean a few other things.
Ideas?

Looks like it's time to replace the upper and lower intake manifold gaskets.
 












replace the intake manifold gaskets - but prior to this

do a "smoke" test, sort of a reverse smoke test, with engine running spray carb cleaner near suspected leaks and see if you can get engine to stumble...I like to know where the leak was so I can check that area for corrosion, and make sure gasket seats correctly.
 






I put a new IAC on it, and reset the codes... No more codes, no more jerking around. Drove 300+ miles so far so good
 






old thread, but continued.. I also had been getting p304 misfire 4 codes -- so.. changed iac, plugs, wires, maf, coil, fuel pump, fuel filter, lower intake gasket, injectors tested, cleaned, new upper intake gaskets, new valve cover gaskets, new fuel rail gaskets, still getting misfire code.. ugh --- did a smoke test (prior to replacing gaskets) and it was leaking smoke from the passenger side rear lower intake area..
 






That doesn't sound right, you said you had a P0304 misfire cylinder 4 code (bank 2), but ended up finding an induction leak on bank 1. Does your scan tool let you look at PIDs? I would be curious to see what percent misfire each cylinder has.
 






I do have the Elm327 Code Read with Torque Pro -- Can get them today. -- I was looking at the values and its a constant misfire on 4. It ran up to 700 misfires real quick. But for specifics, I'll get the exact numbers.
 






Sorry for the delay, I did a smoke test and noticed smoke leaking from the egr valve and egr tube (got new gaskets for those), removed number 4 plug wire and noticed it was broken. So now about to replace plug wires (again) and test.
 






Not sure how to insert photos, but here are the readings.. (btw still misfire 4)

Your fuel trims look good to me.. looks like #4 isn't firing at all. Pull the wire off the EDIS module with engine running and watch for sparks.

Voltage on bank 2 sensor 2 looks out of range, it shouldn't be more than 1 volt.
 






So with it running, I should pull the number 4 plug wire off the coil pack and see if its sparking?
Will do that.
I did test the coil pack (using ohms 20k) red in one coil hole, black in opposite side (gave me a reading of around 13k ohms in all 3 sets)
 






Ok, I'm firing on 4. This misfire doesn't happen all the time, sometimes it runs so smooth. I had a heck of a misfire this morning, popped the hood and noticed the area around number 4 plug was all wet.. Causing misfire due to water getting around plug 4? Where could this water be coming from?
 












I see where you are coming from with that, but I'm just curious as to if it was the head gasket or cracked head, wouldn't I be getting a misfire all the time? Also, never overheated, no coolant in oil.
Also I just replaced intake gaskets, etc -- could I possibly not have tightened something down all the way?
 






Ok, rubbed my finger around down there, seems more like oil than water -- the mystery continues
