I have no idea of the cost since mine was repaired under warranty. The warranty invoice gives no clues on parts cost or labor. They did however list the driveshaft assembly as FoMoCo part # 6L2Z*4602*A.
I did find a New OEM driveshaft for $339.15. Not sure if there are other parts involved...
My 2005 XLT had a vibration at the same speeds you listed. Took it to the dealership and the replaced the rear dirveshaft and balanced the driveline under TSB 05-16-4. This was a warranty repair done at 16900 miles.
