could I have triggered the O/D flash with a quick trans fluid change

`95 Ford Explorer, 172k miles, no previous trans work. 2wd

I have been doing a slow trans fluid change: definition: Extractor removing three quarts, replacing with three quarts, every 6 months. The trans fluid was dirty when I bought this truck at 120 k and have not had a problem. Last week I did this change again and this week I replaced all brake pads and today I was on the highway and the od flash started. THe trans seemed to work fine, o/d turns on/off. I disconnected battery and no code for the remainder of the day.

Could it have been a fluke? I know, I doubt it. Since this is a 1995 it is not obdII but I do have the obd reade for this car. I will check the codes if it returns. Any ideas on additives.

Note; this truck was given to my dad and he uses it locally and not much driving. It is a hauler, beater and is also in the "if it dies I am going to part it out mode" or get a used trans thrown in.
 






At normal cruise, light throttle, 50 mph, cancel O/D push button causes tachometer to jump up a couple hundred rpm?

If yes, same cruise, in O/D gear, give gas pedal just a touch more pressure, not enough to cause downshift, just a touch. If tach. reading increases at all, it is likely the flashing O/D OFF light is telling you the torque converter clutch is not engaging.

If TCC is engaged, the engine speed cannot go up except to follow the increase in vehicle speed, which is minimal if you just push the pedal a little bit more throttle. imp

Edit: If the eng. speed increases as described during the test, it is due to the eng. turning slightly faster than the TC output (or trans. input shaft) due to churning the oil in TC.
 






What does this mean if I trip the code again doing these tests? Would this happen with an overfill? I have been measuring what I take out and precise on total added, and it is a little high. Is there an additive (no I dont believe in magic in a bottle but will ask). Is there a fix for this?

If the car is a local driver can we continue to use? 90% of the time this is a under 45 mph driver.
