When I open/close the drivers door I get a decent "hitch" . You can tell the hinge(s) are wearing out. it's 2007 with 133K miles on it so probably about normal for this part. Initially I looked up on Rock auto and grab some pins and bushings - The bottom hinge doesn't look like it's rebuild-able with the pins and all I've seen online is the part below for like $150+. Is there something I'm missing about replacing the pin in this hinge or is this just a replace and that's all there is to it.

No you aren't missing a thing. I have the same problem. I have a small hole drilled in the top or the cylinder thing so I can lubricare it but it doesn't do all that great. I have thought of looking at other Ford products and seeing if I can find an interchangeable part but as of yet I just put up with the little annoyance.

I have a similar problem on my 07 sport trac. All 4 doors sound like they're going to break off when they open and close. Carmax lubed the doors with lithium grease but it didn't help. They just made a mess.

I thought I was the only one that have this problem on my drivers door hinge. Did you get it fixed thats quite a cost for a hinge.
