Search is your friend, there are several location on the truck to fine the code. I can't tell you for sure on the 04 try looking in the left rear wheel well under the panel. A light and mirror will help. You are looking for a 5 digit number, it will be on a small white (I think) box, numbers will be in large bold print. If not there search for key code location.

wow thank you so much!! i never thought to look ON the truck for the code lol everywhere i found it told me to go to a dealer. and they want like $80.00 just to reset it. thank you very much!!

just too add to whats been said and make it more easy for you .

in the rear hatch ,left side standing in hatch area should be a door panel on the left .open that panel and in there should be a module and some wires more towards the front behind that panel..get a flashlite and look in there .thats where the code should be .unless they moved it on the newer ones?

thank you i will try there as well, i am about to go out and look now. just dont want to tred through the 8 inches of snow lol but i am excited to get the code!

My code was located on a sticker on a black box inside the passenger side right under the seat belt. Take the panel off where the seat belt is and it should be in that general area. Try using a mirror to read the code its easier then trying to pull the whole box out and read it.

Try those two threads to give you an idea of where to look. The one with the pictures is good, but you don't actually need to remove the seatbelt bolt to get to the ECM and see the code, you can just pry gently on the upper portion of the trim panel enough to read the code. I had to do this myself when I bought my '04 Eddie Bauer as it was used as well.

here is a pic of one,

look for the white square. . .that was a number. on the black box to the left just below or next to the 4L2T-. . . .
