I own a 99 Ford Explorer SOHC 4.0 V6. It has 190000 miles on it and typically runs good, however, wehenever it is cold it is difficult to start. We have already replaced the fuel pump and fuel filter and use fuel ingector cleaner at least every two months. Any ideas?

Plugs, wires, cap, air filter.

when you say difficult, what do you mean? Do you mean It just cranks and cranks and then finally starts? When it starts, how does it run right after it starts?

Is it actually the "cold" that causes the issue, or just letting it sit for 4-5 hours (in warmer weather) have the issue too?

Does it start better if you step on the gas a little?

This is bypassing all the generic tune up stuff.. like the plugs/wires etc..
