My first thought was a bad power steering pump, but then found the noise was noisy under the truck, then it comes and goes and as above that it is noisy when standing still in gear and quieter out of gear.

I fear a failing front pump, but there is no leaks, now I am thinking a bad torque convertor....

The transmission shift great 95% of the time.

So what has anyone found with such a noise??

Yes I would check for broken flywheel
Had same problem
Made lot of ching ching noise while idling no noise in gear.or very little
All from underneath truck

I have been told that the 03s had a problem with their torque convertors...

Also mine is the reverse of yours: noisy when standing still IN gear and quieter OUT of gear.

I’ve had the same thing for years sounded a little better after the transmission got rebuilt I just ignore it my exhaust overpowers it now anyways so it doesn’t bug me much. The cats on these cars also like to rattle a ton.
