reaved way up while in drive going slow

Ok, this question has probably been asked before, but I did a search and couldn't find an answer so I guess I'll have to post it. I was pulling out of a parking lot tonight and suddenly my engine reaved way up, probably around 4000 rpm or more. I stopped it and shut it off. I started it back up and once again really high rpm's. I turned it off again and waited a minute and started it back up and it worked fine. Any ideas of what may have caused this or how it can be avoided in the future. Any suggestions would be appreciated. I thought about the MAS or TPS, but wasn't sure if they would cause the problems. Thanks,

also when you first start your car it always revs up high...it is designed that way....4k sounds really high though
 






it doesn't happen every time. It only happened the one time, so I am wondering if it was just a freak thing that hopefully won't happen again. The original tps is on it and it has 125000 miles on it, so I may change that anyway. Just to be safe.
 












Mine did that once, over a year ago now, and it's never done it since.
