Engine whining and surging, and dying a lot

What started out 6 months ago as a loud buzz is now a huge headache. We thought the truck was suffering from a bad IAC, but after 3 replacements I concluded that it must not be that (right?)

Now, it the engine is surging at idle, the loud hum is still there, and it is dying all the time when at idle.

Any ideas?

SOHC V6

**Edited to add - I just went out to check the voltage on the MAF, and it checks out fine. What I also noticed is that once the engine is hot (warmed up), the problem tends to stop somewhat. While I was out there, I couldn't recreate the humming or the dying out while it was in park. If you put it in R or D though, it comes back.
 






what engine do you have? V8? OHV V6? or SOHC V6?

Sounds like lower intake manifold o-rings to me. They're cheap and It will take a few hours to change them. I could do it in 1 and I'm not even American.
