2004 Ford Explorer NBX 4.6 0406 code

Morning,
I have a 2004 4.6 v8 Explorer NBX model. My issue involves a 0406 "sensor circuit a high" code reoccurring.
I initially replaced the EGR valve (with a cheaper one) and this seemed to resolve the engine code popping up (although the car was lethargic at times taking off from stops).
Fast forward a few months and the code kept coming back in. I pulled the EGR valve and found the pipe to the exhaust to be blocked from carbon build up. I cleared the blockage and the tube is clear as I blew through it with an air hose verifying nothing is blocked. I also took the air exchange (where the EGR is mounted on and thoroughly cleaned it, it was filthy!) I reinstalled the EGR and a couple days later the code came back on.
FYI, I also had a new muffler installed a couple months ago and at that time a back pressure test was done and all things tested good.

At this point I am at a loss at what to do.? I have ordered the OEM Motorcraft EGR and will be installing it tonight. I'm hoping this finally resolve the issue.
I've heard a vacuum leak is also a possibility but I'm not populating any codes to support that thought and all other systems (brakes) are working great.

Anyone have any advice or ran into a similar issue that can shed some light I would be grateful.
