95 limited 4.0 buck/miss/skip/stuttering/??

I have done a lot of searching and I have probably missed a bunch, but I STILL can't figure out what's goin on with my Explorer, so now I'm posting to ask.

1995 Limited 4.0 160000 miles. It has a SLIGHT miss at idle, when you pull off from a redlight or driveway, it drives fine but as its climbing it has an intermittent miss to it that turns into an all out lurch/buck/miss/skip around 48-53 mph. If you floor it, it will jump into passing gear, it has a little hesitation but you can feel that it is trying to do what it did 16 years ago without 160000 miles on it. I have checked and checked and checked, vacuum lines, plugs,coil, throttle body,NEW fuel filter,fuel injection treatment, YESTERDAY did Seafoam, and it seemed to get SLIGHTLY better after seafoam but NOT much at all. I'm at a loss, HELLLLLP Please thanks

Also, Autozone put the code reader on it and it said, Error and wouldn't pull any codes it would just say error.

I did the radio code check and it gave me 115 and 125, any ideas Thanks

I have fixed this problem, The Tranny was WAY low on fluid, and the little bit that was in there, was causing the torque converter to lock and unlock irradically. Took the pan out, cleaned the gunk from the screen, had it serviced and fresh fluid put in, and its smooth as glass now. The miss at idle was resovled with some genuine motorcraft plugs and wires.
