A/C Problem - Cold - Warm

I had the blend door actuator replaced about a month ago t the dealer and the system fully charged. The air was blowing perfectly for a few weeks but now is blowing warmer air from the drivers side vents and ice cold air from the passenger vents.

If the blend door was bad and replaced, other than it being bad again, what else could be causing this situation?a

my guess is there still is leak . . .i would get the UV light out at NIGHT and see if you can find any leaks.
