Clogged Cat?

2004 Mercury Mountaineer
Good morning, My 2004 Mountaineer let me down for the second time. V6 2wd.
Could not start.
I have 172,000 miles on it.
At 79,000 miles a cat failed, (clogged?) and the car would not run
for more than a minute or two at idle.

Ford replaced the cat under warranty.

Now the same thing is going on.
Could it be the timing chains or the cats?
The only code is P0128.

Also THE SERVICE ENGINE SOON, light came on about 165,000 miles.
