1998 Ford Explorer XLT installed 3 altnators w/voltage reg. and still receave 19+ volts, re-did all grounds and got my battery out of Caravan 6 cyl only 1month old and works fine in van what else could it be? and check for any shorts on + side and none, also no batt. light and amp gauge is in middle between L & H, Tks.

What did you use to measure the voltage and did you try using it on your other car to check that the meter is good?

I don't have the wiring diagrams for this car but generally a charging system has a wire going to the alternator from the battery that tells the voltage regulator what the alternator's output is. Assuming this charging system works the same way, if that wire had a poor connection it would cause it to drop some voltage across the bad connect and cause the alternator to raise it's output voltage.

V6 or V8?

You didn't post if you have the V6 or the V8. The charging circuit (V6 shown) is fairly simple.

Check the 30 amp fuse in the power distribution box. I don't know the fuse number - find it in your Owner's manual.

Yea, checked with Digital volt meter and to make sure reading correctly compared on van also and it read correctly. I did disconnect regulator connector from alt. yellow stripe and green from there and alt. put out was 12.5 volts, am I missing something.Does the PCM have anything to do with this setup?
