fuel problems

My wife's 09 explorer with a 4.0 v6 sometimes idles rough at startup. I noticed it after fueling it up a few times. She also has driven it on the interstate before and it has chugged or acted like it was going to stall, but then recovered. I can't get it to replicate this. It either does it or it won't. Should I replace the fuel filter and go from there? I know in the 40,000 miles we've had it the filter has never been changed. Is it also possibly the fuel pump itself? Any help is appreciated. Thanks.

The fuel filter is where I would start. Even before this I would check for any trouble codes. I have had a bad coil(s) cause the same issue.
