I searched around a little bit, but couldn't find anything like my problem, so if I missed it and this seems like a question already asked, I apologize.

Anyways, here's my details, I have a 99 XLT, V6 DOHC, essentially stock. The tranny is a rebuilt, and has about 30 000km(19000 miles) on it. Before today I have had no troubles, no warning lights, and nothing that made me think "that just doesn't feel right". It's my daily driver and has only towed an empty utility trailer a handful of times(no other towing since tranny rebuild). So here's my story:
Was driving at a constant speed when the tranny shifted down for no reason(only for a second or 2, and then went back up a gear), was on flat ground and wasn’t trying to speed up. Started going uphill, and it felt like it was surging a little, it would kinda drop off, and then grab again, but the RPM were unchanged. Went off into a little commercial area, Tranny started not shifting, put it up to 5000rpm and it only shifted because I dropped off the accelerator. Pulled over, checked fluid levels, level was good, and color was good (just had a transmission flush maybe 10 000km ago). Put it into neutral, then to drive, accelerated and got nothing. It would start to pull ahead a bit at about 3000rpm, but definitely not grabbing. Put it into 2, and transmission engaged, went to neutral, and then low, transmission engaged. Back to neutral, and then into drive, got nothing. Went to 2 again and engaged, then went into drive, and was still engaged. Started making my way home, “O/D OFF” lamp started flashing, pulled over, shut the truck off, started again and disengaged OD. Started limping home again. Driving with OD disengaged seemed to be shifting alright until a few blocks away from home. I noticed that every time I would pass a car going the other direction, I could hear a squealing reverb off the other vehicle, sounded like a bad brake squeal, but I could only hear it if it bounced off something else. Slowed down to take the last corner before my house, as I took the corner, the squeal got real loud(was not on the accelerator at all). The instant I hit the accelerator, the noise stopped, and so did my transmission, just coasted down the street and parked. Tried all gear positions and got no engagement at all, and had no trouble lamps. Went to tow it and figured we’d try gear positions again, put it into drive, and it sounded and felt like it engaged, hit the accelerator, and got nothing. It now sits motionless in front of my house.

I have a few ideas of what "may" have gone out, but I'm still pretty lost, so I figured I'd ask here as I'm sure that something like this has happened to at least a couple of the users around here.

did you ever figure out what happened? i kinda got the same problem. i have a 99 4wd xlt and i can use 1, 2 and reverse but when i put it in drive i will shift 1 - 2 and then just rev going into 3.... mine happened when i was doing some mudding... also the o/d light was blinking. fluid level is right and nothing esle seems to be wrong. cant figure it out...
 






We are actually just about to rip the tranny out this weekend, we are 99% sure it's the planataries, but we'll find out for sure in the next couple days.
