Lagging throttle response/high RPM at highway speeds/black smoke for first 30 seconds

I have a couple problems. First off my explorer just started spitting black smoke, but it only does it for about the first 30 seconds after i stated it. It stops after that. Took it to a mechanic and he said to clean the throttle body and the mass air flow sensor. Did both of those AND got a new fuel filter. Put injector clean in fresh tank of fuel and its still blowing black.

Second is the lagging throttle response, not sure if the black smoke goes hand in hand with this. When i am at a stop light or stop sign when i go to push the gas it "bogs" down and almost stalls itself out(its an automatic) and doesnt get good acceleration until it hits 1000 RPM.

Third is the higher highway RPM. When i am driving at 110 km/h it usually sits at 2100 RPM(ish) but sometimes it will jerk, almost like it shifts down, and will go up to 2500 RPM. at 100 km/h it sits just under 2000 RPM normally, but again it will shift down or something and will sit at 2300.

If you could help me out that would be awesome!!!
