Lets say I put a 2 inch body lift on my 93 explorer, how big of a pain in the ass will it be to put on a brush guard?
Does the fact that they mount to the frame make it impossible to do w/o serious mods? Do they sell extensions to fix this or am I gonna need someone weld an extension on?

It isn't a pain at all. Most who install a 2" body lift do not raise and relocate the bumper. Most grillguards have enough "wiggle" in the bracketry to lift the guard up an inch before tightening it down, so the grillguard will not look very out-of-line to the headlights. It will still be the same simple bolt-on as on a stock vehicle.

If you raise your front bumper 2", then yeah, you are in for one heckuva mess making that grillguard work. It took me all weekend with complete tools, welder, grinder, etc.

After I did my 3" body, I got a brushguard. The rails that bolt to the frame were very bog on mine, and I had plenty of room to measure 3" above the original holes and drill new ones and it looks fine. But if there isn't enough room to drill new holes, you could just weld on a piece of the same thickness to the rail and drill the holes on that metal.

Don't need to move the brush guard

I installed a 2" body lift and did not need to move the brush guard. Looks good as is.
