Shifting Trouble

I know other people have made posts similar to this one but the problem's weren't quite the same. I have a '94 Sport with a 5-spd transmission. I started out having a little trouble with shifting into first gear. It would act as if there was something preventing the stick from moving forward. The problem has got quite worse and now it takes many tries to get the tranny into first. It is now getting difficult to get second gear and third gear. The other symptom is that whenever I try to put the truck in reverse there is a very loud grinding noise. I've heard things from simply changing the fluid and bleeding the lines to having the entire tranny rebuilt. What should I do? :(
 






The slave or master cylinder are going. Time for a new clutch.
 






Something I forgot to mention . . .

The problem does not exist when the car has been turned off for a while. It quickly gets worse tho as I'm driving (mostly city). Do you think it's the same thing?
