My apologies if this has been answered before, I have looked around for about a half an hour and have yet to read anything which covers all the bases of my current issue.
I have a 1993 Ford Explorer (203k miles) and have recently begun to observe some issues. I will go as in depth into the problem as I possibly can. It started with a simple rough idle and has moved far beyond that over the period of several months. Now when I go out to the vehicle and start it cold, in reverse it will "diesel". My engine when I look at it literally dances inside the engine compartment, this includes being able to feel it when inside the vehicle sitting down. Usually after the vehicle has warmed up the problem is less noticeable, the sputter will still happen at about 2400 RPM's but is nowhere near as rough as the idle. One other indicator could be that my engine seems to be running very very hot, there is no sign of a coolant leak but every month or so the coolant will be completely gone from my system. One thing I have noticed is that my temperature gauge does not work correctly, as it rarely gets beyond the Cold area on the gauge. All of this leads me to believe that I have 2 problems, the first causing the second. The first problem is that my engine is running too hot after warmup, possibly caused by incorrect data being sent from the coolant temperature sensor. The second problem (my guess) is that due to my temperature gauge not working and my vehicles computer recieves incorrect information from one of my sensors (hence, the "diesel"'ing. Im completely lost after this point due to the nature of the overall problem. As stated, once the vehicle is warmed up the problems are much less, but they do still exist. Any help you could give me concerning this issue would be wonderful.

-clogged catalytic converter and / or O2 sensors
-clogged fuel filter or lines
-bad IAC or MAF
-required engine tune up (had one done less than 8k miles ago)
