What are these fuel trims telling me?

Still learning fuel trims and how to decipher them. What direction are they pointing me in? I know something isn't right just not sure what yet.

Air to fuel ratio is too high. Most common cause might be a vac leak, but you could have a dirty MAF sensor, low fuel pressure (bad pump, regulator, or fuel filter), clogged injectors.

I hate to rely on what the apps call things but it also "maybe" looks like your upstream bank 2 O2 sensor (O2S21) isn't reading the way it should (like O2S11) and may need replaced.
