1. radiator fan sounds like a mack truck taking off at times or driving.
2. water temp gauge fluxuates .
3.Fan switch for a/c , and heat , seems only have 3rd, 4th setting. nothing comes out of 1st and 2nd setting.

1. More then likely the infamous radiator leak. I would go and check your fluid levels, having low fluid can cause the PCM to lock up the electric clutch fan.
2. most likely the same as above. The fan is more then likely compensating for the high coolant temps and is kicking into high gear then as the coolant cools the fan slows down and thus the coolant begins to get hot again.
3. I had this issue on mine and it turned out to be a burnt up blower motor resistor. I replaced that and I got my lower fan speeds back.
 






Well, after found out , low on coolant. Water pump is slowly leaking coolant. Radiator looks ok so far. Ordered a new lifetime pump. Resister relay for the fan speed in vehicle comn' next day , after filln resvoir and radiator , suv still made a loud sound like a mack truck... so maybe needn a new clutch fan...... well see after this weekend for water pump install.
 






My Mountaineer makes the loud clutch fan noise as well, radiator was replaced last summer. Been really hot in Dallas all summer and this ties in with the noise on acceleration, should go away when it cools off outside. Running at normal temperature. My a/c control module was bad on my truck, could not lower fan speed or turn it off, very annoying on a 5 hour road trip.
 






Mine always sounds like a mack truck during the summer in the garage...then again I live in Phoenix, Az so I expect it.
