94 a4ld tranny, no reverse, neutral gear

Hi,
We have a 1994 Ford Explorer (A4LD) that only works in the first manual gear. Do not reverse and other gears, only first gear manually.
Is this a serious problem?

Could a damaged vacuum modulator cause this problem?

A vacuum modulator will not be the issue. What is the condition of the fluid? Going to be an internal problem. Good luck
