Hello there I hope this can help you if you are having said issues.
Replaced youre alternator?
And you're battery possibly even the c102a connector behind the alternator this goes for all Ford smart charging systems specifically Ford explorers.
If you go on forscan and are having genplus_fault "yes fault"
And you verified with an oscilloscope (Amazon special) you can actually diagnose the PCM that way.
Still not you're issue? Be very careful at you're own risk get multimeter and probe the alternator case ground and probe the alternator b+ essentially the positive terminal at the alternator and verify for about 13+ volts at idle if this is correct then probe the negative terminal and the positive terminal to the battery directly with a multimeter battery is reading 11-12.6 volts? Proceed to next step connect you're positive lead from the multimeter and ground the negative on the alternator case itself is it reading 13+ 14 volts? If yes grab a pair of jumper cables clip on a jumper cable to the case ground and the other to the negative terminal then turn on the car.
Proceed to look at forscan and Pay attention to genfault_b+ or something like that does it turn off when you ground the alternator if yes you need a new grounding strap not a new alternator or battery you can do this buy getting a very heavy gauge wire something like 2 gauge and jump the alternator case ground directly to the battery negative.
This should fix you're issue I hope this helps someone as it did for me.

If you are not receiving a PWM signal at the harness c102a witch can be aquired from Amazon then something is wrong with the gen com and possibly the moniter line witch 80% of the time it's the harness on the alternator itself the 3 pin c102a
If you are receiving signal you can also check the fuses for the b+ line coming from the alternator to the battery it has a fused link 300 amps and a 150 amp shunt on the battery terminal verify for continuity and no resistance!!
(Provided photo is a reference to what a badly grounded alternator would look like the gen mon is supposed to be almost a flat line but still with a little bumps here and there is also goes way up when you're are at idle to provide current to the system.

Yup and it was still giving me that issue on forscan until I grounded it to the negative terminal
Somewhere in the system there is a faulty ground probably the block to negative ground but none the less I made a new wire with terminals and sure enough it fixed all my issues
