Clunk when starting

My 94 explorer has a clunk coming from the torque converter flexplate area. The torque converter bolts are tight and the trans is tight. The transmission shifts into gear fine and the clunk is only present when starting or at really low idle. I started it today and it shifted into reverse fine but when I hit the gas it refused to move for a few seconds (was slowly inching back instead of going like it should.) After it caught and backed up I let off and it keep reversing like I was still on the gas. The same thing in forward. It then straightened up and did fine. My question is would this be a bad torque converter/flexplate or the trans going out again.
 






Maybe a few stripped teeth on the starter?
 






The teeth on the starter appeared to be fine.
