Engine RPM bounce/chug when letting off throttle at low speeds

For the past few months now I have been having an issue where the engine RPM bounces when letting off the gas. The engine RPM has gone so low that it has stalled once or twice when I hesitated at an intersection or pulling out onto the highway. I have only had this happen at very low speeds or completely stopped. Any advice would be greatly appreciated.
