OD Problem 4th time in two years!

Several questions...
10/15/02 at 137202 miles my tranny gave up. OD light flashing, tranny would not engage above 1000 rpm, finally would not go at all. Had it rebuilt ($1823) with 12 month, 12K warranty. Kens Transmission on Bell in Phoenix.
After one month the same OD light flashing problem again. Took it back. Shop said OD pump or seal bad. Replaced no charge.
One month before warranty up, the same thing happened again. Again replaced no charge.

Now its at 157K miles and the same thing has happened again except this time it is not under warranty.

Another problem may have created this one, but I'll wait for your opinions.

The tranny cooler/radiator seemed to have sprung a leak and the coolant showed tranny fluid in it. Visa Versa for the tranny. Drained it and replaced fluid so that I could get it to a shop to flush it completely. Less than 20 miles later the OD light starts flashing and the tranny won't shift up until 3K rpm or so.

Could water in the fluid have caused this? Would having too much fluid (water/tranny fluid) have caused this? What are the results of having too much fluid in a tranny? Am I just getting bad parts from the shop?
Finally, does anyone in Phoenix know a quality tranny shop?

I was hoping to pull it and work on it myself, but after several months in the garage and work not slowing down, I need to have it done professionally... Baby on the way also makes bucks tight so I don't want to have to go the whole rebuild route again.
