Rear CV axle issue

Over a year ago, I installed a new left rear shackle/bearing assembly on my 03 Explorer Eddie Bauer, V8. Along with that, I replaced the CV axle, the alighnment arm assembly, and ball joints. The other day, the left rear CV came loose in the differential. It left me stranded. What would cause this? I pushed it part way back in today, and was able to limp it around the yard, but I need to know - why would the axle just "fall out?"

Might ought to pull it out for a close inspection of the spline & circlip area. As far as I know, the circlip holds it in. Bad or missing clip, or axle never was pushed in all the way.
