Take the tires off, take the brake caliper off, remove the center cap, remove cotter pin, loosen nut, pull rotor off spindle.
Before puttting new ones on clean/pack bearings with grease, place inner bearing on its race, install grease seal, place rotor on spindle, I like to add a tad more grease from inside the rotor do that if you wish, place outer bearing on its race followed by that big washer, tighten the nut until you stop the rotor from freewheeling, back the nut off enough to line up the castlenut to the hole for the cotter pin (it should now have just a little drag on it), put cotter pin in, replace center cap, replace caliper.
Its been a long time (over 10 years) since I worked on a 2 wheel drive, so forgive me if I forgot something.
