During the summer last year, and this year, I have been running into a progressively worse situation where the truck is now stalling out completely. It does not matter if I have been sitting in Park or Drive, it will die without a moments notice, not to mention a loud hissing noise coming from the engine.

The first thing that I did was to check the vacuum lines and found that there where some that needed minor repairs, and have since been repaired.

Then I pulled the IAC cleaned it the best I could, and then checked the resistance and it reported 9 Ohms which is well within spec (7 - 13 Ohms is Spec)

When I pulled the actuator off I blew through both sides and there was no air getting through at alll; when I put the actuator back on, it then started allowing air to pass with some resistance, but not enough to be confortable.

I then re-installed it, ran the truck in drive with the AC OFF and in Drive, and then ran it again with the AC ON and in Drive. It seemed to be better, but not enough to keep me from having to put it in neutral and give it gas.

Any ideas?

If you have to give it gas to prevent it from stalling, then more than likely its your IAC. I would replace it anyways since its a 91. But I would also check the fuel pressure regulator to be safe and also clean the MAF.

Can you suggest a means of cleaning the MAF? Probably wouldn't hurt to change out the fuel filter as I have no idea if it has been replaced before I bought it.

Use electrical contact cleaner on the MAF. Do a search on cleaning the MAF and it will yield results. Definately change the fuel filter. Don't forget to get the special tool needed to take the fittings off the fuel filter.

check the seal between the mass airflow and the intake tube/air box. make sure your motor mounts are intact. if motor is twisting when in forward gears pulling on those two above mentioned areas then once paced into a neutral gear allowing them to set back and re-seal {so to say}. . this is a common failure on older exploders and very overlooked. the motor mounts that is. . hope you can make sense of this ..
