1. Yes.
2. Every 10-20K miles or so.
3. Same as Jimbo: cross the fronts to the rear and move the rears straight forward.
4. Though I don't rotate often or on a regular schedule, I do get a 4-wheel alignment every couple of years.

I got 105,000 miles off the last set of tires on my X (Michelins rated at 80K).

1 - Yes I rotate them
2 - 5,000 miles
3 - I cross rotate my tires (front tires go to the rear crossed and rear tires go straight to the front)
4 - N/A
5 - 10,000-15,000 miles or sooner if I feel vibration at highway speeds
How about you Aldive?
I got 4 new tires(Dueller A/T Revos) about a year ago(11,900 miles) and its amazing how fast during the first few months the tread lowers, guess thats part of new tires breaking in....at least they are all wearing evenly and losing tread at a much slower rate

4) i rotate, its stupid not to, you wind up with sinething like the car on my lift right now. 06 ES330, 17K on the odo, both front tires are beyond destroyed

1) yes

3) depends on the type of tires and the treadwear on the front. If there is feathering on the front tread I do a reverse cross rotation so the feathered edges backwards on the rear tires so LF -> RR, RF -> LR, RR -> RF, LR -> LF. I figured this out on a 90 sport that I had, after rotating the tires and getting a horrible hum from the back ones, so I swapped them from side to side in the rear and it cleared up the problem and I have been doing it ever since. As long as the tread is not directional.

5-There are four of them, they are stable and don't need to be balanced.

2) about every 6k to 10k
3) Criss cross.. Front to rear.. rears to front while swapping sides
4) sometimes they dont' get rotated for a while.. don't always have time
5) Never.. no wheel weights.. I use air soft pellets in the tires to keep them balanced (once you hit 40 they are well balanced)
interesting.. with that little tidbit of information I may just purchase and mount my own tires next time. The only thing that kept me from doing it was the balancing.
