vacuum leak symptoms but cant find the culprit. help!

Its a 97 4.0 ohv 2wd. I'll start with I've just had to do everything on the top end so that means heads gaskets as well as every other gasket on the top end. I had the same problem before the head gaskets as well. On cold start ups it idles low then immediately goes up like it should and idles down. On warm starts, on the other hand, will start up and immediately rev up like its a cold start then idle down in about 10 seconds and then hesitate pretty bad at the proper idle. It'll hesitate then it'll rev up to about 1200rpms ON ITS OWN about 30 seconds later and drops back down and idles fine. When I'm going into any gear, drive or reverse,
the rpms jump to about 1200 and when I go from in gear to neutral or park it'll jump to around 1500. I've looked for vacuum leaks with carb spray and even plugged all of the external vacuum lines (evap, fuel oressure regulator, brake booster, etc) and nothing! The intakes arent leaking at all either, brand new gaskets. It also has a LOT of rev hang and revs up on its own if i even just slightly tap the gas. Ive also replaced/ cleaned just about everything IAC is new, tps checked out good, mass air flow is cleaned, fuel filter has about 2k on it, fuel pressure is good, no codes and the list goes on. I decided to reset the computer and all that does is make it run like complete crap. It'll hesitate beyond belief and smell pretty eheavy in fumes from the exhaust while its doing it. when i did the headgaskets the exhaust valves looked a little more burned than normal, coukd this be clogged cats? Im about stumped now so please please help me!!

Forgot to add that after the warm starts, if I don't let it rev up on its own and idle back down and just put it in gear while its hesitating it is way down on power and smoothes itself out as I'm driving
 






Bump! Need help!
 






After driving a few days a code for bank 1 and bank 2 system too lean just came up
 






no magic answers. any vacuum line or hose connected to the motor can me the source of a leak. add to that intake manifold gaskets, power brake booster, PCV hoses, elbows and grommet, HVAC controls and hoses, vacuum ball/reservoir even a crack in the air intake hose. probably the best way to find a vacuum leak is with a smoke machine.
 






Oddly enough when I unplug my IAC valve all of the issues go away. Like ALL of them. Maybe I got a faulty part?
 






Its an auto zone brand I believe. I've tried multiple IAC valves with all the same results
