hey guys got all i need to do a body lift (2 inch) only i dont have a steering shaft extension, some say you dont need and others say you do. is this true? and if only some do why is that and how can you tell if your x needs one

I have 1994 explorer xl. when accelorating from a stop the trans seems to have a hard time going into 3rd gear, it revs up to 3000-4000 rpm then i let the gas and then it goes into third. the tranny is still strong and not slipping or anything, it just does not like to go into 3rd. any ideas...
