I have a 98 EFI that has gotten progressively worse. Sometime in December it threw the usual both banks are running lean / rich. I followed the suggestions stating clean the MAF and IAC and cleared the code. No real symptoms returned. About 3 weeks ago it started having a rough idle and rough acceleration from about 1500-2000 RPMs. I replaced the plugs and fuel filter at that point thinking that would maybe cause some of the problems that it was having. The symptoms hadn't returned until a couple of days ago. Extremely rough idle and rough acceleration. It feels like it is missing, but the CEL had not come on indicating a misfire. I replaced the IAC this morning seeing if it would solve the problem. It has helped with the idle in park. It finally threw a code this aftenoon P0174. I went ahead and replaced the plug wires and cleared the computer, which didn't help either. Upon running the vehicle for a little bit I noticed what appeared to be smoke coming from near the intake manifold and coil pack.

It still runs extremely rough, any suggestions?

I have this exact issue. Did you ever figure it out?
 






Take the serpentine belt off and run it and see if it idles better and spin your pulleys to make sure they're working right
 






Well. Actually, the part I'm referring to is the smoke that came from the coil pack area. My coil pack overheated and melted internally which cause the misfire and then it just died
 






I would swap out the coilpack just to see.
 






I did swap the coil pack and it overheated and fried within a few minutes of sitting there with the key on. I could tell it was fried (at least afterwards I was able to tell) because the engine started to misfire again. Then the coil pack smoked a minute later and melted internally. I got code P0352 as well.
