annoying check engine light, and codes p0153, p0156

ok, here is the scoop. evil is living up to her name......again. lately it has been showing a check engine light, but will not stay on constantly. it will come on for a few days, then go for a few days, then come back on, then go off. it is realy pissing me off (that why i call her evil!) so anyways my buddy has a snap on scan tool and scanned it for me. it read the following:

p0156-h02 sensor low voltage of down stream. h02 is less then desired bank 2

p0153-ho2 sensor slow response. response rate of h02 up stream h02 sensor is slow

now back up a few months back. i did a sea foam treatment to it by pulling off one of the vacuum tubes and pouring it in there, then poured the rest of the can in the tank. as well as i cleaned the k&n filter. when i pull the vacuum tube off (the one by the filter box) the light came on (vacuum leak). did worry about it, cause i knew why it triggered the light. it went off after a few days. then a month later, it came back on. figuring the oil from my k&n fowled the MAF sensor, i pulled that out and cleaned it. it went off a few days later. a few weeks later, it came on again, so once again, i pulled the MAF and cleaned it once again.
now i had a problem like this last year, scanned it, and it said bad up and down stream sensors, bank 2. so a bought 2 bosch sensors and replaced them.
this truck has about 101k miles on it. in the last year i have changed the plug wires, 2 02 sensors, cleaned the MAF and throttle body cleaned the k&n twice. i also changed the plugs about 20k miles ago as well. there are no vacuum leaks that we can find, no exhaust leaks either. the truck runs good, with lots of power, starts fine as well, but there is some sut on the edge of the tail pipe.
i do think the remaining 02 sensors are the factory ones, but is bank 2 the drivers side, or the passengers side? i searched and found one thread that says that bank 2 is the drivers side, and we changed the ones on the passengers side (buddy was positive that was the correct side). help me out before i spend $150 on two sensors that i may not need.
it is a 98 SOHC BTW.
